Summer Camp 2026 Registration

I certify that I am the participant or the parent and/or legal guardian of the participant indicated below
(“Participant”). I consent to the Participant’s participation in the Nashville Academy of Fencing Inc. (the “Club”). I understand and acknowledge there are inherent risks in participating in any athletic activity at the Club and the use of any club equipment that could result in loss, damages, personal injury or death. These risks include, but are not limited to, bruises, cuts, transmitted illnesses or diseases, strains, sprains, neck/spinal injuries, broken bones, cardiovascular injuries, dehydration, sunburn, concussions or other bodily injuries. I acknowledge and understand the risks of exposure to contagious diseases, including COVID-19, to me, Participant or other family members by Participant’s attendance at the Club. I understand the Club follows preventative measures and protocols for COVID-19 in place compliant with state and local requirements and I agree that Participant will follow those requirements as well.

Nonetheless, I acknowledge, even with such measures, there are risks of infection to COVID-19 to me, the Participant or other family members by Participant’s attendance at the Club. Participant and I knowingly and voluntarily assume all risks associated with his/her participation in the Club. In consideration for Participant’s participation in the Club, Participant and I knowingly, voluntarily, and forever waive, release and discharge the Nashville Academy of Fencing, Inc. and its officers, directors, members, employees, and coaches (the “Releasees”) from all present and future claims of any type for any harm or loss, including property damage, personal injury, illness or death, that either Participant or I may incur while at the Club, regardless of fault. Participant and I agree to indemnify, hold harmless and covenant not to sue the Releasees for any claims, damages, personal injury, illness, death, medical expenses, disability, lost wages, loss of capacity, property damage, court costs, attorney’s fees or any other losses or claims of any kind arising out of Participant’s participation in the Club. I represent and warrant that Participant is physically and/or mentally able to participate in the Club and no physician or other health care provider has advised me or Participant otherwise. I further understand that and give permission for photographs to be taken of Participant during the Club and that the Nashville Academy of Fencing, Inc. may use these photographs for publicity and/or advertising, with the understanding that Participant’s identity will not be disclosed in conjunction with any such publicity or advertising.

I HAVE READ AND UNDERSTAND THIS DOCUMENT AND ACKNOWLEDGE THAT IT LIMITS OR EXTINGUISHES CERTAIN LEGAL RIGHTS THAT PARTICIPANT AND I MAY HAVE AGAINST THE RELEASEES. I UNDERSTAND AND AGREE THAT THIS CONSENT, WAIVER, AND RELEASE OF LIABILITY IS BINDING UPON PARTICIPANT, ME, AND OUR RESPECTIVE FAMILY MEMBERS, HEIRS, EXECUTORS, ADMINISTRATORS, ASSIGNS, AND ANY OTHER PERSON WHO PURPORTS TO ACT ON OUR BEHALF.

Nashville Academy of Fencing requires credit card payments for services provided to members. Nashville Academy of Fencing uses the GymDesk application to securely store credit card information for the express purpose of billing club members for services. Nashville Academy of Fencing does not have access to your credit card number or the security code. Only you have access to this information.

By adding a credit card to Nashville Academy of Fencing's I authorize Nashville Academy of Fencing to invoice me for club membership and services. I understand that this information will be saved for use in future transactions. I understand that this authorization will remain in effect until cancelled and the terms may be updated periodically and that I will be given notice of such updates by email. I understand that I may cancel this authorization at any time by removing my credit card information from the system, or by requesting Nashville Academy of Fencing to remove my account. By signing this authorization, I understand that my membership will automatically renew for another month/session and I will be charged unless I request a cancellation. Requests for cancellation not made at least 10 days before renewal may be subject to a charge back fee of $40.
